Community Outreach Specialist. The NWCO VNA's Aging Well Program is seeking a Community Outreach Specialist to support comprehensive healthy aging in western Colorado. This full-time position will introduce and integrate Healthier Living Colorado, a statewide chronic disease self-management program, in 24 counties across Colorado's Western Slope. The position is responsible for understanding and promoting this 6-week workshop for adults with chronic conditions. The duties and responsibilities include recruiting, training and supporting individual trainers/leaders and participants of the workshop across the Western Slope. The successful candidate should possess excellent marketing, communications, customer service and administrative skills, be familiar with community health programs and have a passion for improving health in rural Colorado. Candidates should demonstrate specific experience successfully working in collaborative, professional partnerships with multiple organizations. Extensive travel within Colorado may be required. To work in a busy health center as part of a medical team to provide high quality care to patients, evaluates the needs of the patients, respond to patient inquiries and performs a medical triage function. Must be able to effectively perform the tasks and procedures required of a Medical Assistant in a primary care clinic setting. Also perform some administrative functions related to the clinic.
